STEP 1
Collect and prepare to upload all required Registration Documentation listed below:
Parent/Guardian License/State Issued ID
Original Birth Certificate with a raised seal
Current physical and immunization record (dated within the last 12 months)
School Record Request Form. Please print and sign and send it to the sending school and to the Milton Public Schools at registration@miltonps.org (Only required for grades 1-12)
Complete Proof of Residency. Residency checklist can be found here.
If you are unable to upload documentation, please drop it to the drop box located to the right of the front entrance of Milton High School, 25 Gile Rd.
STEP 2
If your student has any of the following documents, please email a copy to registration@miltonps.org.
Copy of Individual Education Plan or 504 Plan. Must be signed by both the parent/guardian and the sending school
Custodial Agreement
Please download and complete this paperwork and email it to registration@miltonps.org
Step 4
You will receive an email from MPS with next steps.
